RMA Request Form
We require that any product returned to MCWW for warranty have a Return Material Authorization (RMA) number.
Before returning any product, you must obtain an RMA number from MCWW and complete an RMA authorization form.
Write the RMA number clearly on the outside of the shipping package, and include a copy of the form inside the package.
All shipping costs to MCWW are assumed by the customer.
